POLST (Physician Orders for Life-Sustaining Treatment)
CHCD helps lead a regional coalition to ensure that a new POLST process for clarifying seriously ill patients'
treatment decisions becomes recognized and commonly used throughout the region. The POLST form complements an advance directive by turning a patient's
wishes into actionable medical orders.

Convened by Eskaton, the Coalition is a multi-disciplinary, multi-organizational group of physician leaders and representatives
from emergency medical services, hospitals, skilled nursing facilities and other healthcare and community organizations.
During 2011 and 2012 the Coalition will be working to:
- heighten the visibility of POLST and end-of-life issues to the public through its speakers' bureau
- provide educational opportunities for health professionals and staff from acute care, skilled nursing, EMS, home health and hospice settings
- help ensure that high quality POLST conversations are the standard of practice throughout the region

Statewide work
CHCD has worked with the Coalition for Compassionate Care of California in its promotion of POLST:
- March 2011 - wrote an issue brief
on a regional collaborative effort to improve advance care planning and end-of-life care in Sacramento area nursing homes.
- March 2009 - produced a white paper
highlighting the work of eight community POLST coalitions and presented results at CCCC annual conference.
